I/We understand that Mortgage Intelligence Inc. collects personal information in accordance with its Privacy Policy to provide the services requested, better understand my/our financial needs and determine how Mortgage Intelligence Inc. and affiliated companies may be of service to me/us.

The information collected may include name, address, contact information, income, employment, age, net worth, investment objectives, insurance coverage and banking information; unique identifiers where authorized by law; and information from a consumer reporting agency or other source concerning account information and creditworthiness.

I/We understand that Mortgage Intelligence Inc. may share personal information, as detailed in its Privacy Policy, with brokers or authorized agents and with organizations such as credit bureau agencies, financial institutions, private investors and insurance companies as needed to determine eligibility for products and services or provide services requested.

I/We understand that a Social Insurance Number, when provided and permitted by law, may be used as an aid to identify me/us with credit bureaus and financial institutions and for credit history file matching.

I/We declare that the information provided in the mortgage application is a true and complete representation. I/We authorize Mortgage Intelligence Inc. or its designate to obtain a credit report and, where needed, additional credit reports or credit monitoring for up to six (6) months from the date signed for the purpose of securing credit or related products and services.

I/We acknowledge that the Mortgage Intelligence Inc. Privacy Policy is available for review and that collection, use and disclosure of personal information will be carried out in accordance with that policy.

I/We agree that a photocopy or electronic copy of this Consent and Privacy Agreement has the same value as an original. I/We have read, understood and received a copy of this agreement.
